HSV Isolation In Cell Culture Has Been The Diagnostic Gold Standard For Many Years

This review describes advances in cell culture-based viral diagnostic products and techniques, including the use of newer cell culture formats, cryopreserved cell cultures, centrifugation-enhanced inoculation, precytopathogenic effect detection, cocultivated cell cultures, and transgenic cell lines. The microscopic examination of the unstained cell culture monolayer has long been the standard approach for detecting viral proliferation. Although herpes simplex virus (HSV) may produce easily visible CPE within the first 24 h of incubation, most viruses demonstrate CPE only after 5 to 10 days of incubation, with some, such as cytomegalovirus (CMV), averaging 10 to 30 days for CPE production (Table (Table2). There have been many recent advances in diagnostic techniques for HSV infections, including new viral detection methods and serological tests. Tube culture isolation is the traditional gold standard for HSV detection and the reference method against which all other tests are measured (16,17). Detection of HSV antigens has been achieved in fixed cells by DFA tests or immunoperoxidase tests on fixed, solubilized cell specimens (24-26). This is the first comparison of HSV PCR with two other routine diagnostic methods for confirming genital herpes infection in a symptomatic population.

An enteroviruses from the CSF, or HSV from a genital lesion is highly diagnostic for the infection. Although PCR has been the diagnostic standard method for HSV infections of the central nervous system, until now viral culture has been the test of choice for HSV genital infection. However, HSV PCR, with its consistently and substantially higher rate of HSV detection, could replace viral culture as the gold standard for the diagnosis of genital herpes in people with active mucocutaneous lesions, regardless of anatomic location or viral type. Virus isolation in cell culture has long been regarded as the diagnostic gold standard and is the diagnostic method of choice in the United Kingdom.

Animal vectors for human HSV infections have not been described, and humans remain the sole reservoir for transmission to other humans. Isolation of HSV by culture remains the definitive diagnostic method of documenting an HSV infection, including establishing neonatal HSV disease. HSV CNS disease (113), it still has been replaced as the gold standard for virologic confirmation of CNS involvement by PCR due to markedly greater sensitivity of the PCR assay, as discussed below. In this study, we compared viral culture and the amplification of HSV DNA by the polymerase chain reaction (PCR) with respect to sensitivity, cost, clinical utility, and turnaround time. Recently, HSV infection has been implicated as an important cofactor in the acquisition, transmission, and possibly even the progression of the human immunodeficiency virus infection (2, 7, 8). Laboratory diagnosis of genital herpes has traditionally relied upon HSV cell culture as its gold standard (9, 10, 11). A hundred selected specimens and many clinical specimens were tested for methods comparison and assay validation. Various PCR-based methods have been introduced to the clinical laboratory, for example, conventional PCR, nested PCR, and PCR-restriction fragment length polymorphism (RFLP). Patients and SpecimensSpecimens used in this study were classified into two groups: 100 specimens including already known as positives for clinical validation and clinical specimens obtained from patients with signs and symptoms suggestive of HSV or VZV infection for diagnosis.

Is There Still A Need For Viral Diagnostic Laboratories To Perform Virus Isolation?

Genital herpes due to HSV-1 (through oral to genital transmission) has also become common; HSV-1 is a frequent cause of primary genital herpes. HSV isolation in cell culture has been the diagnostic gold standard for many years. This method is considered gold standard for virus isolation and identification 2. It has been reported that in ocular scrapping this method has to cell culture. THe stability of DNA relative to viable virions or antigens make these methods appropriate for specimens that must be in transit for long periods for time or at ambient temperature. In humans, recurrent ocular HSV-1 disease has been attributed most to use of prostaglandin agonists, corticosteroids, and inhibitors of angiogenesis. Therefore, a long-term (one year or greater), low-dose oral antiviral prophylaxis is recommended for patients with a history of recurrent HSV stromal keratitis. Isolation of HSV-1 by cell culture is considered the gold standard in laboratory diagnosis of HSV epithelial keratitis.
